MDT Home Health Care Agency, Inc. 91 PERICARE PURPOSE: Clients are assisted with perineal care, as appropriate, to clean perineal area, provide comfort and control perineal odor. Gloves shall always be worn when providing perineal care. RFSPONSIBLE PERSONNEL: RN, LPN, HHA EQUIPMENT: Wash cloth, basin, soap, towel, gloves PROCEDURE: 1. Explain procedure to patient. 2. Wash your hands and don gloves. 3. Provide patient with privacy 4. Position patient comfortably on back. 5. Cover patient with light blanket or sheet. 6. Wash, rinse and dry front section of patient's perineal area cleaning from front to back. 7. Position patient on side. 8. Place towel along patient's buttocks. 9. Wash, rinse and dry back section of perineal area 10. Massage coccyx area with lotion. 11. Assist client with comfort measures. 12. Remove equipment from bedside. 13. Clean/dispose of equipment, as appropriate. 14. Place soiled linen in laundry. 15. Remove gloves and wash hands. DOCUMENTATION: Document care provided and patient tolerance of procedure. Document any skin irritations and if further action is indicated, document follow-up/notification of clinical supervisor and/or physician. ____________________________________________________
