MDT Home Health Care Agency, Inc. 114 SKIN CARE PURPOSE: Patients are assisted with skin care to clean and condition skin, augment patient's comfort and maintain/increase circulation. RESPONSIBLE PERSONNEL: RN, LPN, HHA EQUIPMENT: Towel and washcloth, basin, lotion PROCEDURE: 1 Explain the procedure for skin care to the patient. 2. Wash your hands. 3. Provide patient with privacy 4 . Position and drape patient according to which skin area will be given care. 5. Wash the skin area with warm, soapy water. Rinse and dry thoroughly. 6. Massage reddened skin areas gently and surrounding skin area more briskly with lotion. Do not massage briskly over bony prominences. 7. Any reddened areas that do not disappear within 30 minutes must be reported to clinical manager. 8. Assist patient with comfort measures. 9. Remove, clean and store skin care equipment. 10. Place soiled linen in laundry. 11. Wash hands. DOCUMENTATION: Document care provided and patient’s tolerance to procedure on Clinical Visit Note. Report any skin irritations to clinical manager for appropriate follow up. ____________________________________________________
