MDT Home Health Care Agency, Inc. POLICY ON OUTCOME AND ASSESSMENT INFORMATION SET (OASIS) ENROLLMENT POLICY: Our Home Health Agency, shall in compliance of Federal conditions of participation, enroll in collect and report the Outcome and Assessment Information Set, (OASIS) data. We shall to incorporate the OASIS data, using the language and groupings of the most current approved version of OASIS, into our comprehensive assessments. Comprehensive assessment of patients . Each patient must receive, and our HHA will provide, a patient - specific, comprehensive assessment. For Medicare beneficiaries, the HHA must verify the patient’s eligibility for the Medicare home health benefit including homebound status, both at the time of the initial assessment visit and at the time of the comprehensive assessment. Reporting OASIS information Encoding and transmitting OASIS data. Our HHA must encode and electronically transmit each completed OASIS assessment to the CMS system, regarding each beneficiary with respect to which information is required to be transmitted (as determined by the Secretary), within 30 days of completing the assessment of the beneficiary. Accuracy of encoded OASIS data. The encoded OASIS data must accurately reflect the patient’s status at the time of assessment. Transmittal of OASIS data. Our HHA must: (1) For all completed assessments, transmit OASIS data in a format that meets the requirements of data format. (2) Successfully transmit test data to the QIES ASAP System or CMS OASIS contractor. (3) Transmit data using electronic communications software that complies with the Federal Information Processing Standard (FIPS 140–2, issued May 25, 2001) from our HHA or the HHA contractor to the CMS collection site. (4) Transmit data that includes the CMS - assigned branch identification number, as applicable. Data Format. The HHA must encode and transmit data using the software available from CMS or software that conforms to CMS standard electronic record layout, edit specifications, and data dictionary, and that includes the required OASIS data set. Our Agency shall to collect, start of care, resumption of care, follow-up, discharge to the community, transfer to an inpatient facility (with or without discharge), and death at home, for Medicare beneficiaries or Medicaid beneficiaries older than 18 years old and with skilled services. A Non-OASIS type SOC assessment will be used in all Non medicare beneficiaries, including HMO patients, waiver program (non Medicare recipients). To encode the information, in safe, timely, accurate and confidential manner and to be sure that the encoded OASIS data must accurately reflect the patient's status at the time of assessment our Agency will use “QIES ASAP System” or like software from CMS OASIS contractor, and in compliance, the designated employee (OASIS Coordinator), will be trained in the use, and Home Health Agency Policies A-75
