MDT Home Health Care Agency, Inc. which will be given to the PI Staff for use in overall Performance Improvement (QAPI) Activities. 2. The OASIS measures will be assessed. Rates which are lower than the norm/standard for the agency will be analyzed and reviewed for improvement. 3) OBQI/QM data is downloaded from CASPER system. Results are reviewed/analyzed by designated staff. Reports given to Administration, Clinical Manager and may result in changes in QAPI activities. All of OASIS assessments, with the exception of transfer to inpatient facility and death at home, require the clinician to have an in-person encounter with the patient during a home visit. The transfer to an inpatient facility requires collection of limited OASIS data (most of which may be obtained through a telephone call). STATEMENT OF PATIENT PRIVACY RIGHTS: Our Agency and any staff acting on behalf of our HHA in accordance with a written contract will ensure the confidentiality of all patient identifiable information contained in the clinical record, including OASIS data, and may not release patient identifiable OASIS information to the public. As a home health patient, our patient have the right to know why we need to ask the questions, and be informed that their personal health care information will kept confidential, also they have the right to refuse answer the question, and look at any time, by previous request, their personal health information. All admissions process will include the CMS OASIS’s STATEMENT OF PATIENT PRIVACY RIGHTS, which will be left at Client’s home for further consult/review by Patient or Patient’s family. Also our Patients will be informed about the Authority for Collection, Principal Purposes for which their information is intended to be used, and effects if they do not provide the information requested. The CMS’s PRIVACY ACT STATEMENT-HEALTH CARE RECORDS, is also included in our Admission procedures, and will be left at Patient’s home for further reference. For Patients who don’t have Medicare/Medicaid coverage, al needed information will be available to them, included Federal/State requirements for collection, purposes and use of the information, and remark that we keep anything we learn about them confidential, the CMS’s NOTICE ABOUT PRIVACY, will be incorporated in our Admission process, and left at Client’s home for further reference. Our Agency shall be up to date with all new/further regulations, updates, or changes in OASIS collection/encoding process, by attend In-services, review Posted Regulations, or trough CMS communications. Our OASIS coordinator, shall be informed/trained with all up to date OASIS documentation/regulations/in services, and comply with the following mandatory schedule: Home Health Agency Policies A-78
