Turn down bath blanket and drape patient’s chest and abdomen with towel. 8. Place a bath towel beneath the patient’s arm closest to the caregiver. Moving from the wrists to shoulder, wash and dry arm using even, smooth strokes. Dry arm and apply deodorant. Repeat process on opposite arm. 9. Wash and dry the abdomen. Then turn back the bath blanket to cover the patient’s chest, abdomen and groin area. 10. Uncover the leg closest to you and place a bath towel beneath it. Bend leg at the knee and wash and dry it from the ankle to the hip using smooth, even strokes. Repeat the process for the opposite leg. Wash both feet and dry and apply lotion to feet. 11. DO NOT massage legs or calves. 12. Cover patient with bath blanket. Change bath water and gloves. 13. With clean water and a clean washcloth, wash and dry genital area using a different section of the washcloth for each downward stroke. Always wash from front to back toward anal area. For male patients, folds of skin at head of penis should be pulled downward to complete cleaning. After cleaning, foreskin should be pulled upwards toward tip of penis. 14. Perform indwelling catheter care according to MDT Home Health Care Agency, Inc. policy. 15. Change water, gloves and use clean wash cloth. 16. Turn patient to side or stomach. Bathe and dry back and buttocks, moving from shoulder to buttocks. Massage back with lotion, giving attention to bony prominences. 17. Bathe and dry the rectal area from front to back. Wash hands and change gloves. 18. Apply clean gown and other articles removed before the bath. 19. Assist client with oral hygiene. 20. Remake the bed or change linen and remove bath blanket. Wash hands. 21. Place a towel beneath the patient’s head, brush and comb hair. 22. If the patient has a hospital bed, return bed to initial position and position patient comfortably. Return side rails to original position. Wash your hands * When leaving bedside to change water, make sure to raise side-rails to original position. DOCUMENTATION: Record activities performed on the Clinical Visit Note. If further action is indicated, document follow-up/ notification of clinical manager and/or physician.
