MDT Home Health Care Agency, Inc. 14 INFORMATION REGARDING SERVICE OUTCOMES POLICY: Clients are informed of the outcomes of services provided, including unanticipated outcomes, as they occur. PURPOSE: 1. To ensure client/family are knowledgeable about outcomes of services. 2. To enable client/family/cg to participate in current and future decision affecting care/services. PROCEDURES: 1. Client/family/cg are informed of outcomes of services needed to be able to make decisions regarding current/future care. Information is documented by nurse/therapist and filed in client record. 2. Client/family/cg is informed of unanticipated outcomes as they occur. 3. Administrator/designee is responsible to inform client/family /cg of unanticipated outcomes that relate to sentinel events. 4. Client/family/cg may not be aware that an unanticipated outcome has occurred; therefore it is of the utmost importance that they be informed, as this may impact on their decisions regarding current/future scare/services. 5.
