MDT Home Health Care Agency, Inc. 31 CONDOM CATHETER APPLICATION PURPOSE: Condom catheters will be applied as per physician’s order to control urinary incontinence, to prevent irritation and breakdown, to reduce embarrassment and to encourage a return to activity. Catheter should be removed daily. RESPONSIBLE PERSONNEL: RN, LPN, HHA with documented training where allowed by state licensure regulation EQUIPMENT: Gloves, soap, basin, towel, washcloth, condom catheter, drainage bag, skin prep (optional) SPECIAL CONSIDERATIONS: A condom catheter is commonly used by males who are experiencing urinary incontinence. The condom provides for some measure of control over the incontinence, while reducing the risk of urinary tract infection frequently associated with indwelling catheters. Patients and caregivers need instructions regarding the safe application of the condom, the mechanism for attaching the device to a drainage system and principles of skin care associated with its use. Specific teaching needs will vary individually, and should be assessed considering the overall condition of the patient, the reason for use, the length of time to be worn and equipment available. Knowledge of the patient’s frequency of incontinence will help establish guidelines for length of time during the day the condom needs to be worn. PROCEDURE: 1. Explain procedure to patient. Wash you hands. Put on disposable gloves. 2. Position patient to expose genitalia. Assess penis and scrotum for evidence of excoriation, swelling and irritation. 3. Cleanse shaft of penis with soap and warm water. Rinse. Dry thoroughly. 4. Roll the condom catheter over the penis. Leave 1-2 inches between end of penis and the catheter. 5. Examine for tightness. Fit should be snug, but not constrictive. Do not twist condom. 6. Secure according to manufacturer’s directions. Be sure that condom is not restricting blood flow to the penis. 7. Connect the adapter urine receptacle (i.e., leg bag, beside drainage bag) according to manufacturer’s directions. 8. Remove and store used equipment. 9. Instruct caregivers in application of condom catheter, to inspect site frequently for discoloration or swelling and to release and reapply constricting band if swelling or discoloration occurs.
