MDT Home Health Care Agency, Inc. 45 FEEDING PURPOSE: Patients will be assisted with feeding, as appropriate, to meet the patient's nutritional needs. RESPONSIBLE PERSONNEL: All personnel PROCEDURE: Avoid confusion and unnecessary noise at mealtime. Foods should be served at the appropriate temperature. 1. For the person who can feed self: a. Position the patient as indicated. Elevate to a sitting position when possible. b. Allow the patient to wash hands. c. Place the meal in the most convenient position for the patient. (1) Assist the patient as necessary. (2) Place eating utensils and various dishes within easy reach. 2. For the patient who cannot feed self: a. Attempt to diminish the patient's feeling of helplessness. For example, (1) Be seated at bedside if this is convenient. (2) Tell the patient what is available, if he or she cannot see. b. Fill the fork or spoon only half full. c. Alternate foods as the patient desires. Allow patient choice d. Leave the patient comfortable. 3. For the blind patient who can feed self: a. Give the location of the items of food, using the numbers of a clock as a guide. b. Have the patient use the back of the hand to locate carefully the coffee cup, soup bowl, silverware, plate, glass, etc. c. Have all foods ready for the patient salt and pepper to add (as preferred), meat cut, and beverage available. DOCUMENTATION : Document care on clinical visit note. Include portions consumed. If further actions is indicated, document follow-up/notification of clinical manager and/or physician. ____________________________________________________
