MDT Home Health Care Agency, Inc. 61 INFUSION THERAPY ADMINISTRATION PURPOSE: Administration of infusion therapy will be performed in accordance of physician’s orders and the infusion therapy protocols. All medication orders should be verified for name, dose, route, time and duration of the administration. All medication/solution labels should be checked for patient’s name, drug, dose, route, solution, infusion duration and time, expiration date and checked to ensure label is clear and correct. Please refer to the “First dose” policy as needed to ensure adherence to all infusion policies. The clinician will be knowledgeable in the administration of medications/solutions via bolus, cyclic, intermittent and continuous methods, the specific drug being administered, infusion devices utilized for administration and compatibilities and complications prior to the initiation of the infusion. All medication/solutions will be administered at room temperature. All solutions will be used or discarded within 24 hours from the time the container is opened, with the exception of medication cassettes and tubing utilized for continuous infusion. Gravity administration includes intravenous piggyback anti-infectives and other medications/solutions of a fluid volume of 500 cc or less (unless contraindicated be physician or manufacturer’s recommendation). Cyclic and continuous administration of medications/solutions should be achieved via a volumetric infusion device. Use of a flow-regulating device will be determined by the therapy to be administered and clinical condition. The appropriate size filter will be used only as indicated by the drug therapy and/or according to drug and equipment manufacturer’s guidelines. A needleless system is the preferred connection for all IV devices. PERSONNEL RESPONSIBLE: RN EQUIPMENT: • Prescribed medication/solution • Intravenous administration set (with filter if indicated) • Needle/access system injection set/cannula • Alcohol swabs • Normal saline-filled syringe or normal saline flush solution • Heparin-filled syringe (if applicable) or heparin-flush solution • Tape • Electronic infusion device (if applicable)
