MDT Home Health Care Agency, Inc. 109 SHAMPOO CLIENTS CONFINED TO BED PURPOSE: Clients are assisted with hair shampoo, as appropriate, to clean client's hair, massage client's scalp and enhance hygiene . RESPONSIBLE PERSONNEL: RN, LPN, HHA EQUIPMENT: Shampoo, conditioner, wash basin, wash cloth, plastic sheet/bag, Chux Pads, comb, brush, and/or pick, hair dryer (as requested) PROCEDURE: 1. Explain the procedure for hair shampoo to client. 2. Position client with head and shoulders moved to the edge or head of bed in a comfortable position. 3. Place plastic or improvised water trough, made from rubber sheet built up on both sides by rolling a towel into each side, under client's head so water constantly drains. 4. Thoroughly wet client's hair and scalp with water at a comfortable temperature. 5. Apply shampoo to hair and work it into a lather. 6. Rinse shampoo from hair. 7. Towel dry and comb client's hair. 8. Remove equipment. 9. Assist patient with comfort measures. DOCUMENTATION: Document hair shampoo results, date, hair and scalp condition, client's needs, outcome, etc. on the Clinical Visit Note. If further action is indicated, document follow- up/notification of clinical manager and/or physician. ____________________________________________________
