MDT Home Health Care Agency, Inc. 110 SHAMPOO CLIENTS NOT CONFINED TO BED PURPOSE: Clients are assisted with hair shampoo, as appropriate, to clean client's hair, massage client's scalp, and enhance comfort. RESPONSIBLE PERSONNEL: RN, LPN, HHA EQUIPMENT: Shampoo; conditioner; towel; comb; brush; hair dryer (as requested) PROCEDURE: 1. Explain the order and procedure for hair shampoo to client. 2. Position client in a comfortable position with head hanging over sink. 3. Thoroughly wet hair and scalp with water of a comfortable temperature. 4. Apply shampoo to client’s hair and work it into a lather. 5. Rinse shampoo from hair. 6. Towel dry and comb client's hair. 7. Remove and store equipment. DOCUMENTATION: Document hair shampoo results, date, hair and scalp condition, client's needs, outcome, etc. on the Clinical Visit Note. If further action is indicated, document follow- up/notification of clinical manager and/or physician.
