MDT Home Health Care Agency, Inc. 138 URINAL PURPOSE: Clients are assisted with urinal use, as appropriate, when they are unable to use bathroom facilities without assistance. RESPONSIBLE PERSONNEL: HHA, RN, LPN EQUIPMENT: Urinal, gloves PROCEDURE: 1. Explain procedure for urinal use to client. 2. Wash your hands. 3. Provide for client's privacy. 4. Assist client with placement of urinal. 5. Elevate head of bed as necessary. 6, Assist client with urinal removal 7. Assist client with pericare as necessary. 8. Assist client with comfort measures. 9. Dispose of contents of urinal. 10. Wash your hands. DOCUMENTATION: Document voiding results and pertinent observations made in the Clinical Visit Note. ____________________________________________________
