MDT Home Health Care Agency, Inc. 145 WEIGHT PURPOSE: Home health care staff members weigh patients, as appropriate and as ordered, to assess patient’s nutritional status and fluid retention. RESPONSIBLE PERSONNEL: RN, LPN, HHA, PT, OT EQUIPMENT: Scale PROCEDURE: 1. Explain the procedure to the patient. 2. Wash your hands. 3. Provide the patient with privacy. 4. Weigh patient on standing scale and assist patient as needed to ensure stable. 5. Read and record patient’s weight. 6. Wash hands. DOCUMENTATION: Document weight, date and time and any pertinent observations during the procedure, on the Clinical Visit Note. If further action is indicated, document follow-up/notification of Clinical Manager and or Physician. ____________________________________________________
