MDT Home Health Care Agency, Inc. 151 Z-TRACK INTRAMUSCULAR INJECTION PURPOSE: All irritating medications (e.g. iron dextran) will be given using the Z-track technique to deposit medication in deep muscle layers and prevent leakage into subcutaneous tissue. Medications are administered in accordance with specific physician orders regarding dosage and frequency. The First Dose Administration policy should be adhered to. SPECIAL CONSIDERATIONS: 1. This technique is recommended for any irritating medication (e.g. iron dextran (Imferon), and where deep intramuscular injection is desired. 2. A two-inch needle must be used to reach deep muscle layers in an average patient. RESPONSIBLE PERSONNEL: RN, LPN EQUIPMENT: Alcohol swabs, syringe, two 2-inch needles, medication, needle box PROCEDURE: 1. Prepare the injection as in the “Intramuscular Injections” procedure but change to a sterile two-inch needle after drawing up the solution. 2. Draw .3 to .5 ml of air into the syringe. 3. Hold skin tightly in place with the non-dominant hand. 4. With the dominant hand, inject the needle deeply into the muscle. 5. Inject medicine and air into the muscle. This should be done slowly. 6. Remove the needle and release skin held by non-dominant hand. 7. Wipe site with alcohol pad. Assess the site. 8. Reposition and cover the patient. 9. Dispose of syringe and needle in sharps container. 10. Wash hands. DOCUMENTATION: Document on the Clinical Note the time, date, medication, route, dose, injection site used and any pertinent reactions or side effects. If further action is indicated, document follow-up/notification of Clinical Manager and/or physician.
