MDT Home Health Care Agency, Inc. The Agency's system assures the following internal and external informational management functions: 1. security, integrity and confidentiality of all information used 2. accessibility and timely access to internal and external data bases and information networks to be used by clinical, management and administrative staff 3. uniformity in data collection and reporting systems 4. knowledge-based information and literature are maintained in a systematic and organized manner and can be easily retrieved for administrative, management and staff needs 5. ensures that data and record content is complete accurate, timely and in accordance with Agency policy, and when applicable, law and regulation 6. coordination of appropriate patient/client information exchange among referral sources, staff and other team members, contractors, physicians, other direct care providers, third party payers external data base and knowledge based resources, as required by the Agency's management and patient/client care needs 7. qualified personnel enter data timely, and accurately, and all coded data is periodically reviewed to validate the integrity of data entry and reported findings 8. availability of information-management function which provides for the definition, capture, analysis, communication, reporting and/or use of comparative performance data and information (QAPI), as it becomes available and is applicable 9. systems for monitoring and tracking timely preparation and submission of medical orders, clinical documentation, statistical and management data as required by Agency policy, and state law and regulation Home Health Agency Policies A-98
