MDT Home Health Care Agency, Inc. 164  Evaluates the patient’s response to the therapy plan of care in a timely manner as required by Agency policy and applicable laws and regulations and coordinates care and/or provision with other members of the healthcare team.  Reassesses the patient to determine the ongoing needs for therapy care, treatment and/or services and progression toward goals as stipulated by Agency policy. • Medical Social Services:  Evaluates the social, emotional, economic and environmental needs of the patient and his/her family in his/her place of residence. (within 48 hours after order received)  Evaluates the patient’s/family’s response to the social services plan of care in a timely manner as required by Agency policy and applicable laws and regulations and coordinates service provision with other members of the healthcare team and appropriate community resources.  Reassesses the patient and his/her family to determine the ongoing needs for social services and progression toward goals as stipulated by Agency policy. • A Registered Nurse completes all initial evaluation assessments utilizing the Outcomes and Assessment Information Set (OASIS) except in those instances where the physician has ordered only therapy services. (within 48 hours after order received) • The Registered Nurse regularly reevaluates the patient’s nursing needs utilizing OASIS data items. • If the physician orders only therapy services, it is acceptable for the appropriate therapist (Physical Therapist or Speech-Language Pathologist) to complete the initial assessment evaluation utilizing the OASIS, or the therapist may complete a therapy specific assessment form with the nurse completing the OASIS data set within five (5) days of the start of care date. • Physical Therapists, Respiratory Therapists, Occupational Therapists, Speech- Language Pathologists and other disciplines assigned to the patient should assess and reassess the patient in his/her respective areas of training and expertise in conformance with applicable scopes of practice, state and federal laws and regulations and accreditation standards. • Experienced Pediatric Nurses should assess, evaluate and provide care to pediatric patients.
