MDT Home Health Care Agency, Inc. ADMINISTRATIVE RECORDS AND REPORTS Financial Records Financial records, including patient/client billing files, shall be retained for five years, or for six years following the month in which the cost report to which the records apply is filed with the intermediary, whichever is longer. Records are confidential and shall be protected from damage and unauthorized use. Personnel Records Personnel files shall be maintained for three years following the employees termination from services. Records are confidential and are protected from damage and unauthorized use. Administrative Records, Minutes and Reports The Agency shall maintain the following administrative records and reports: . A list of the Board of Directors and titles . A list of any Agency Committee members and titles . A list of stockholders and percent of ownership, if indicated . A list of the Budget Committee members . A list of Record Review Committee members (QAPI) . Minutes from all committees (retained for at least 5 years) . Agency policy manual . Agency procedure manual . Operating budget . Capital expenditure plan, if indicated . Accounts payable and receivable ledgers . Payroll records . Annual program evaluation . Certificate of incorporation . License to operate . Bylaws, Minutes of all official meetings of the governing body are retained for a minimum of five (5) years. . Contracts . Personnel list . Patient/client roster . Incident reports .
