MDT Home Health Care Agency, Inc. 211 MEDICATION ERRORS PURPOSE: • To identify elements of risk for the occurrence of medication errors, and implement areas for improvement within the medication management system of MDT Home Health Care Agency, Inc. • To minimize the occurrence of medication errors, thereby promoting the achievement of positive patient outcomes while the patient is receiving care, treatment and/or services from MDT Home Health Care Agency, Inc. • To promote patient safety through a standardized process for managing and reporting medication errors, and developing an action plan of correction. • To comply with applicable laws and regulations pertaining to medication management. • To comply with and implement the industry’s most current best practices. POLICY: • MDT Home Health Care Agency, Inc. has instituted a process to respond to actual or potential medication errors. • All potential or actual adverse medication events requiring notification through external state, federal, USP or FDA channels will be reported by MDT Home Health Care Agency, Inc. in compliance with applicable laws and regulations. POTENTIAL MEDICATION ERRORS: • Potential Medication Errors May Include Any of the Following: • “Near miss” such as:  Medication is labeled incorrectly, i.e., wrong name, dose and/or route of administration; error recognized and medication not administered to or ingested by patient  Medication orders transcribed incorrectly; no error in administration  Incorrect medication; no error in administration ACTUAL MEDICATION ERRORS: • Actual Medication Errors May Include Any of the Following: • Medication dose omitted, with or without harm to the patient
