MDT Home Health Care Agency, Inc. 212 • Not ordered dose administered to/ingested by patient • Wrong medication and/or dose administered to or ingested by patient, and/or wrong route of administration with:  No harm to patient  Increased monitoring required, i.e., increased number/frequency of visits but no change in vital signs and no harm to patient  Increased monitoring required; change in vital signs but no ultimate harm to patient  Need for treatment with another drug, transfer to acute care setting for intervention to prevent permanent impairment or damage  Permanent patient harm  Patient death PROCEDURE: • When an actual or potential medication error occurs, the prescribing physician/licensed independent practitioner and MDT Home Health Care Agency, Inc. are to be notified immediately by the clinical staff member who identifies the error. • An Incident Report is to be completed and forwarded to Administration. • Documentation will include the name and dose of the drug, the patient's response, time of physician notification and any orders received. • Actual or potential medication errors will be tracked and trended as part of the performance improvement activities of the MDT Home Health Care Agency, Inc. Summary reports of findings, conclusions, actions taken and recommendations for improvement will be generated by the Performance Improvement Department/Committee. Summary reports are submitted to the Professional Advisory Committee and/or Board of Directors.
