MDT Home Health Care Agency, Inc. VERIFICATION OF PHYSICIAN AND PROFESSIONAL STAFF LICENSE PURPOSE: To establish a procedure to verify and document that our professional staff and the physician giving orders for care to a patient has a current and clear license to practice in the state in which the agency is located. GENERAL STATEMENT: Professional staff are Registered Nurses, Licensed Practical Nurses, Therapists, and Social Workers. (Home Health Aide and CNA the Agency will verify the require training hours, before assign any client). A physician is a doctor of medicine, osteopathy, or podiatry legally authorized to practice medicine and surgery by the state (s) of: Florida POLICY: The agency will verify professional staff license at initial application and a time of renewal thereafter (every two years), and referral physicians license when make the first referrals to the agency, and bi- annual thereafter. PROCEDURE: . Human Resourcesmaintain the professional staff license verification control list. On application process the human resource staff must verify staff license in the State designated web site. Copy of the State on-line verification results, will be place in the staff human resources chart. Non clear license will be discussed with the affected professional, and no client will be assigned until clear verification is received. Thereafter, at the time of staff license renewal process, the license will be verified in the State License verification web site, within 30 days of the renewal requirement date. · Manager of Intake maintains a list of physicians’ names and license numbers. This list is updated regularly as new physicians make referrals for service. · Upon admission of a patient, the Manager of Intake checks the list to verify that the referring physician is licensed and clear. · If the referring physician’s name does not appear on this list, the Manager of Intake staff will verify the license number by telephoning the State Board of Medical Examiners. · Verification shall be documented on the verification log, including the following: physician name, who the license is verified with, date verified, and name of staff member doing the verification. · If we receive several referrals from the same physician, at least every two year his/her license will be verified.
