MDT Home Health Care Agency, Inc. CONSENT PRACTICES PURPOSE: To provide a consistent process for obtaining authorizations for treatments and related services. POLICY: Authorizations are obtained prior to the initiation of treatment from the patient/client or legal representative in a manner that is consistent with state law. PROCEDURE: 1. Treatment authorizations are obtained prior to the initiation of care from either the patient/client or a legal representative. 2. Treatment authorizations address the following items: A. The nature and purpose of the proposed care B. The probable risks and probable benefits of proposed care C. The availability, if any, of treatment alternatives and the attendant risks and benefits of the same. D. The risks of foregoing proposed care and alternative forms of treatment. E. Informed consent F. Proposed frequency/duration 3. Treatment authorization practices are modified to meet state regulatory requirements. 4. A request is made to the patient/client or legal representative to involve family caregivers in the consent process. 5. A consent document is signed by the patient/client or legal representative, including where appropriate, the authorization to involve family caregivers in the consent process.
