MDT Home Health Care Agency, Inc. CHANGE OF CONDITION ASSESSMENTS Policy MDT Home Health Care Agency, Inc. will conduct a comprehensive assessment as frequently as a patient condition warrants due to a major decline or improvement in patient status, the comprehensive assessment will reflect the patients current health status. Procedure 1. The agency's criteria for determining a major decline or improvement include: a. A significant change in functional status (3 or more ADL's) b. Development of a significant condition such as pressure sore, weight loss, enteral tube feeding or 3 of the following reasons: i. A new diagnosis ii. Initiation of 2 or more disciplines iii. Addition of 2 or more medications iv. An increase of 3 or more visits per week 2. The criteria for a SCIC adjustment include an event: a. Not envisioned in the original plan of care. b. Requiring an oasis assessment resulting in a new HHRG and c. Requiring the receipt of a new physician's orders reflecting the significant change in treatment. 3. If the new HHRG is lower and the Patient experienced unanticipated improvement a SCIC is required. If the new HHRG is higher, the agency has the option to submit a SCIC.
