MDT Home Health Care Agency, Inc. COMPUTER USAGE POLICY In certain instances, MDT Home Health Care Agency, Inc. has provided employee with, or access to a computer in order to assist employee in performing certain employment-related tasks, if Protected Electronic Patient’s records is accessed all HIPAA guidelines must be followed at all time. These computers are the property of MDT Home Health Care Agency, Inc. and their usage is governed by the following policy: MDT Home Health Care Agency, Inc. reserves the right: · To monitor all computer files, electronic mail (''e-mail''), Internet access, and computer usage of employee · To access and disclose all computer information to others at any time without prior notice to the employee. · To override all passwords (affording no security protection for the user) without prior notice · To automatically store e-mail messages on the computers backup system, and the “message delete'' function does not restrict or eliminate the ability to review e-mail correspondence of the employee · Change regular user Password twice a year. All Internet activity will be monitored regularly and if any obvious abuse is observed, limited access will be enforced. Employee may be given one written warning of Internet abuse before Internet privileges are revoked. The following constitutes prohibited computer activities: A. Computers may not be used for personal purposes unless authorized by MDT Home Health Care Agency, Inc. This includes but is not limited to e-mail, the Internet or word processing. B. At no time should anyone other than the assigned employee use the computer or laptop in the home. The laptop should be transported in a carrying case at all times; do not expose to extreme temperatures; prevent exposure to water or other liquids. The employee will not load or attempt to load any software programs to the laptop. If the laptop is damaged or lost due to willful neglect or willful abuse, the employee will be financially responsible for repairs and/or replacement up to the full cost of the laptop. C. Encryption passwords and similar devices are not allowed unless authorized by MDT Home Health Care Agency, Inc. The employee is responsible for keeping his/her unique identifier confidential. The employee should not use another employee's unique identifier under any circumstances. This will be looked at as a breach of confidentiality and the employee could be dismissed. D. Communication, including but not limited to e-mail, Internet usage, and word processing, consisting of profanity; gossip; personal attacks; derogatory remarks; threats; sexually explicit material; slurs pertaining to race, national origin, handicap, religion, sexual orientation, and gender; sexually harassing language; pornography; or any other communications which are discriminatory, defamatory, unlawful or otherwise inappropriate are strictly prohibited. A VIOLATION OF THIS POLICY MAY RESULT IN DISCIPLINARY ACTION UP TO AND INCLUDING TERMINATION. Home Health Agency Policies A-123
