Fee change from Medicaid to Medicare: discharge and re-admit patient 2. Retro, fee changes should be avoided. If necessary, change date to actual date or beginning of episode. Utilization Management must first review retro fee changes beyond current episode. 3. When a patient is being fee changed from Medicare to another payor, the clinician needs to complete: a. ABN b.
