MDT Home Health Care Agency, Inc. OUR PHILOSOPHY The Agency's goals and services are based on two fundamental philosophical principles the belief in the innate worth of the aged and disabled individual and the belief that each individual, regardless of age, race, color, creed, sex, sexual orientation, national origin or handicap(s) is entitled to maximize his potential as a human being and as a member of society. It is the contention of the Agency that the aging process is a normal state in the development of any individual and that chronic disease and disability are, to some degree, a part of that process. This Agency is dedicated to rehabilitating aged and disabled individuals within the confines of their residence, in order that they may maximize their contributions and fulfill their goals as a family member and member of society with a minimum of conflict. In accomplishing this end, it is felt that the self-respect of the individual can and will be enhanced. The Agency also is a vital member of its community. As an employer, The Agency practices non-discrimination and strives to provide opportunities for personal and professional growth. As an integral part of the business community, The Agency makes every effort to serve the people with attention to current area practices and specific needs. The agency believes in the concept of health as defined by the World Health Organization as “a state of complete physical, mental and social well being, and not merely the absence of disease or infirmity.” This agency is committed to the philosophy that individuals in need of skilled nursing care and other home health services should have access to such treatment at home unless there is a compelling medical/social reason prohibiting such care. Care is provided without regard to race, creed, sex, age, national origin, disabilities, or sexual orientation. Services are provided to individuals within the constraints of the agency’s resources. The agency’s philosophy is contained in the following principles: · Quality home health care should be designed to meet the unique needs of the individual patient and family. · Home health care should be tailored to assist the individual and family to achieve the optimal level of health. · All home care will be provided based upon our respect for the dignity and worth of the individual and the family. · Each patient should have access to the provision of appropriate health care services in the least restrictive setting. · Each patient has a right to be informed of the services available to him/her and to participate in the planning of his/her own health care. The philosophy of MDT Home Health Care Agency, Inc., is to offer to our patient the support with our commitment of quality care, abiding by one of the most emphatic principles on which this agency is founded and that is unconditional love toward every patient we come in contact with. Thus we consider our care total, meeting the physical, emotional, psychosocial and mental needs of the patient. This is reflected in the way our care plan is developed and contact time is managed with the patient. We also bring into perspective the immediate family setting and the background of the patient in drawing up a care plan and thus when necessary involve them in the patients overall care. We are aware of the fact that home health services are cost-effective and a better alternative to institutionalization for a large part of the population in need of medical, nursing or supportive services. We therefore are willing to support and avow to its principles and guidelines. This agency is also open to receiving suggestions, opinions and constructive criticisms in order to dynamically, improving service for our patients. We also ensure that every member of our nursing staff and other medical personnel sent to care for patients are well qualified. Based upon the foregoing, MDT Home Health Care Agency, Inc. will adhere to the following criteria in the provision of services to our clients: 1. MDT Home Health Care Agency, Inc., will see that our clients receive quality health care of the highest standards. 2. Every potential employee will be interviewed in depth, and careful checking of references will be carried out. Home Health Agency - - Introduction 1
