MDT Home Health Care Agency, Inc. 3. The Registered Nurse, Licensed Practical Nurse, and Home Health Aide will be required to present his/her current license, a copy of which will be made and kept in his/her file, at time of hire and at each renewal date thereafter. 4. Verification of all licenses will be requested from the State Board of Nursing. 5. All new employees will attend an orientation, which shall include but not be limited to: a. Job descriptions for the different categories b. Employees' expectations of MDT Home Health Care Agency, Inc. c. MDT Home Health Care Agency, Inc.'s expectations of its employees d. Policies and procedures e. Orientation on the facilities served by MDT Home Health Care Agency, Inc. f. In the case of Home Health Aides, a written test. 6. MDT Home Health Care Agency, Inc., will keep an up-to-date copy of the State's Nurse Practice Act in the office and will adhere to its standards and requirements. 7. The Administrator and the Director of Nursing, Clinical Manager shall evaluate each employee. 8. MDT Home Health Care Agency, Inc., shall provide ongoing in-service education for all employees (IE: refresher course at each year-end on topics relevant to our current cases). 9.
