MDT Home Health Care Agency, Inc. to patients paying through personal funds or private insurance who default on their contract through non-payment. Our agency provide social worker assistance to patients to help them determine their eligibility from government funded programs if their private funds have been depleted or will be depleted. In any order of change of frequency received by the Agency we will inform the patients, in compliance with their rights, the new frequency changed by our Agreement Addendum form or any other applicable form.
