MDT Home Health Care Agency, Inc. skills testing at time of hire and annual, validation of malpractice coverage for independent contractors, validation of completion of the orientation process, validation of signed and dated confidentiality statements, validation of in-service/continuing education participation as applicable, validation of exit interview, immigration and naturalization statement I-9, a signed Affidavit of Compliance with background screening, results of background screening, and dates of employment and separation from the agency. Evidence of continuing education and in- service training for home health aides shall be on file, and available for inspection within 3 hours of request. The file shall be maintained by the Director/Administrator/Human Resource personnel, and contents of the file shall be accessed by the employee on his/her request, unless the employee signs a waiver for such an open policy. 20. All files belonging to this agency, its personnel, staff, patients and clients, are confidential and will be treated as such. An employee shall not have access to the file of another employee. Personnel who, by reason of their duties, have access to the files of patients, clients or other employees, shall treat all such information as confidential and on no account shall disclose any information so obtained to any other person. A violation of this policy shall constitute grounds for immediate dismissal. 21. Only the Director/Administrator of this agency has direct access to personnel files and records. 22. A complete personnel record shall be maintained on each employee. After Termination of Employment, the Agency shall to maintain a full Employee Chart for at three (3) Year after that termination. 23. The secretary, in collaboration with the Director, shall be responsible for implementing and coordinating personnel policies and procedures. 24. Our Agency is a Drug Free Work Place, a Drug screening test may be required for each suspect employee, for use/abuse of illegal drugs. 25. Criminal Background Check is required for all field employee/contractor, all conducted through AHCA. 26. Each field direct care personnel must receive a minimum of 2 hours of training, in- service in HIV and AIDS, the training should include universal precautions and infection control procedures to ensure proper practices are followed. Also must have a 2 hours Alzheimer’s disease training as state regulation. 27. If we change the Administrator or alternate, we shall notify AHCA Home Care unit office prior to or on the date of change, the notification shall consist of submission of the person’s name, professional resume, license, if applicable, and a copy of the Affidavit of Compliance with Criminal Background, the Administrator will submit also the level 2 screening. 28. Our Agency shall maintain a record of the employment or contractual history of all agency personnel, both employed or under contract, and shall make submission of such history a condition of employment or contract, we rely on history provided by applicant. 29. The opportunity for exit conference are offered to all terminating employees, this exit interview are documented and trended as applicable. Home Health Agency. - - Personnel/Operations Policies B-10
