MDT Home Health Care Agency, Inc. APPLICATION FOR EMPLOYMENT PROCESS POLICY The Home Health Care Agency processes application for employment in consistent, specified manner. PURPOSES To enable applicants for Home Health Care Agency’s employment to give information regarding their clinical qualifications, professional qualifications, work experience and performance. To collect consistent and sufficient information form each prospective Home Health Care staff member. PROCEDURES 1. Prospective Home Health Care staff members are requested to complete and submit all paperwork as outlined in the Application Instruction Manual, as required by Federal, State, and Local governing authorities, regardless of past employment with the Home Health Care Agency. a. Test If applicable to status of applicant (i.e. RN, LPN, HHA, PT, RT, Homemaker, Companion). b. Application Paperwork: Contained in application packet. c. Verification of Status: License, Certification, Diploma if applicable. d. Federal and State required forms: Tax forms (W-4 Form, W-9 Form, W-2 Forms, 1099 Form, as applicable), I-9 Form, etc. e. The Agency reserves the right to perform background investigation (including police record) to all applicants within five days of application date. 2. Personnel folders for all employees and contractors shall include the following: a. Application Form b. Name and address applicant. c. Social Security Number. d. Date of Birth. e. Employee/Contractor Agreement (If Applicable) f. Tax Exempt Form (If Applicable) g. Transportation Agreement Form Home Health Agency. - - Personnel/Operations Policies B-12
