The Home Health Care Agency shall maintain a record of the employment history of all Home Health Care Agency’s personnel, both employed or under contact, shall make submission of such a condition of employment, and shall verify the history unless through diligent efforts such verification is not possible, pursuant to s. 400.497 (1)(c), FS also 1. Reviews the submitted application paperwork forms to verify that they are complete and accurate. b. Request addition information from perspective staff member if the forms are incomplete. c. Assist prospective home health care staff member with disabilities in completing forms, as necessary. d. Submit the completed application forms and folder to the Director of Nursing, Clinical Manager of Home Health Care to determine clinical applicability. 4. The Director of Nursing, Clinical Manager of Home Health Care: a. Reviews the completed application forms for clinical applicability. b. If the appropriate vacancy exists, has the Field Staff Supervisor arrange for an interview. c. If the appropriate vacancy does not exists, has the Field Staff Supervisory notify the prospective Staff Member that the application will maintained on file and considered when an appropriate vacancy becomes available. 5. Employment applications are treated as confidential personnel information. 6. The Home Health Care Agency Manager is responsible to ensure that only those applicants who have completed all requirements for Federal, State and Local governing authorities are utilized as Home Health Care staff members.
