MDT Home Health Care Agency, Inc. POLICY ON HIRING/REFERENCES, AND RETAINING EMPLOYEES Subject to our existing policies on hiring, requiring non-discrimination on the basis of race, religion, disability, national origin, sexual orientation, age or sex, the following criteria shall also apply: It is further the policy of this agency that at least one valid reference check by telephone shall be adequate for purposes of confirmation and eligibility for hiring. A sample Reference form for potential employees (see Human Resources form) shall be used in all such instances. Faxed completed references by the previous employer is acceptable. MDT Home Health Care Agency, Inc. desires to hire qualified candidates for all job openings. To ensure that individuals who join MDT Home Health Care Agency, Inc. are well qualified and have a strong potential to be productive and successful, it is the policy of MDT Home Health Care Agency, Inc. to complete appropriate background checks of all employees who provide direct patient care. PROCEDURE: Prior to an offer of employment, the hiring Manager should ensure all candidates for employment are properly interviewed and the following background checks are completed: A minimum of two(2) references (Prefer two work and one personal/school if 2 works are not available) Verification of license or certification; within 5 days of after/before the expiration date (if applicable) Former employer references should be obtained on all candidates with prior work histories being considered for employment. Verification should include at least: Dates of employment Position held Salary or hourly wage rate Whenever possible, the actual job duties and performance should also be verified. After a conditional job offer has been made, the following background investigations shall be completed: Criminal records check List of Medicare Program Excluded Individuals from the list maintained by the Office of the Inspector General (OIG). Where there is no prior employment, references should be obtained from school, churches, or personal associations. Reference and background investigations are strictly confidential and become a part of the Employee’s personnel file. MDT Home Health Care Agency, Inc. will comply with federal, state, and local laws regarding criminal records of Employees employed by nursing homes, home health agencies and hospice, or other health care agencies in which the company may provide service. The following offenses disqualify an applicant from employment: Murder Arson Manslaughter, aggravated manslaughter of an elderly person or disabled Home Health Agency. - - Personnel/Operations Policies B-26
