MDT Home Health Care Agency, Inc. POLICY AND PROCEDURE EMPLOYEE ORIENTATION The orientation process includes a description of position(s)/qualifications of the individual(s) responsible for conducting orientation activities, the Director of Nursing, Clinical Manager is assigned as responsible for all Clinical staff orientation (in the case of Therapists Orientation a help of a qualified registered Therapist must be used), and the Administrator or qualified individual for the orientation of all non-nursing staff. Non-nursing-staff employees (or relocated staff) will be required to undergo an office orientation. During the orientation period (to be conducted and supervised by the Administrator), employees will familiarize themselves with office location, other staff members, equipment and supplies, existing policies and procedures and their respective job responsibilities. In the absence of the Administrator, the Director of Nursing, Clinical Manager or any other appropriate designate will supervise the orientation. No test is required at the end of the orientation, but a signed statement by both the employee and the supervisor attesting to the successful completion of the orientation will be required. New clinical staff employees (or relocated staff), will undergo an orientation, to be conducted and supervised by the Administrator or (in his/her absence) the Nursing Director. During the orientation the new clinical staff will familiarize herself/himself with agency procedures and policies, the Nursing Manual, the organizational chart, and duties and responsibilities as defined in the Job Descriptions section of this Manual. All new clinical staff must review our educations/orientation materials on measurement of blood pressure, cardiopulmonary resuscitation, all new nursing staff will be required to read in detail our policy on Infection Control. A written/competency practical test is required at the end of the orientation, and a "Compliance Checklist" must be completed and signed by both the clinical staff and the qualified same discipline supervisor (in Professional staff)/DON, Clinical Manager. A competency test is required of all Home Health Aides, we will maintain documentation of the aide’s successful passage of the competency test, which consist in two parts: a practical part (administered and evaluated by a registered nurse or licensed practical nurse under the supervision of registered nurse), in which competency is determined through observation of the performance of tasks, and the written part with questions to answer. The orientation policies and procedures include, but are not limited to: • Review of the individual’s job description and duties performed and their role in the HHA • Organizational chart • Mission statement • HHA philosophy • Record keeping and reporting • Confidentiality and privacy of Protected Health Information (PHI) • Patient’s rights • Advance Directives • Conflict of interest • Written policies and procedures Home Health Agency. - - Personnel/Operations Policies B-31
