MDT Home Health Care Agency, Inc. POLICY AND PROCEDURE EMPLOYEE HEALTH POLICY Home Health Care staff members are expected to comply with Home Health Care Agency employment requirements. PURPOSE: To identify pertinent health problems of Home Health Care field staff members. PROCEDURES 1. A Home Health Care field staff member who has accepted employment with the Home Health Care Agency must submit a physical health examination signed by a physician. a. Requirement that, prior to contact with patients all direct care field employee/contractor must submit the result of a PPD Mantoux method tuberculin skin test (TST) or Xray screening. The CDC does not recommend serial chest x rays. The employee should carry documentation of a baseline chest radiograph results and the positive test result for M. tuberculosis infection to us. For HCW (healthcare workers) whose initial TST results are negative, a second TST is not needed if the HCW has a documented TST result from any time during the previous 12 months. If a newly employed HCW has had a documented negative TST results within the previous 12 months a single TST may be administered in the new setting, but not required. For those workers who had a previous TST within the last 12 months and it was negative, our agency will not request another TST at time of hire. b. All direct care field Employee/contractor must submit an annually statement from a health care professional with verification through a TB symptoms screening tool or PPD c. Signed by a licensed physician, based on an exam with the last twelve months, that the field employee/contractor is in reasonable good health and does not appear to be at risk of transmitting communicable diseases. d. If a field employee/contractor is found to have, or is suspected of having , a communicable disease he/she shall be removed from duties that require contact with patients until certification is received from a duly licenced physician that such risk no longer exists. For state employees/ contractors, these requirements shall be met using departmental medical staff. e. A completed Health Questionnaire Form (See Health Questionnaire Form). f. A completed Hepatitis B Declaration Form and Questionnaire Form (See Hepatitis B Declaration Form and Hepatitis B Vaccine Questionnaire Form). g. A completed and signed Physical Examination Form (See Physical Examination Form). 2. The home health agency is responsible to ensure that field staff continue to appear to be in good health and that the patients are not placed at risk by employees/ contractors with positive tuberculin skin test (TST). 3. Documented field employee health information is held in strictest confidential. 4. Completed field employee/contractor health records are filed inside the personnel files (confidential envelope) 5.
