MDT Home Health Care Agency, Inc. INVOLUNTARY TERMINATION OF EMPLOYMENT POLICY The Home Health Care Agency consistent processes Home Health Care staff members involuntary termination of employment, following appropriate agency policies and procedures. PURPOSES 1.To address Home Health Care staff members inappropriate behavior. 2. To facilitate the processing of the termination of Home Health Care staff members employment with the Home Health Care Agency. PROCEDURES 1. Definition Involuntary termination: The termination of a Home Health Care staff member who has not volunteered to terminate employment with the Home Health Care Agency and who has demonstrated critical inappropriate behavior, regardless of whether the staff member has previously demonstrated inappropriate behavior or been disciplined. 2. Action a. The Director of Nursing, Clinical Manager of Home Health Care 1. Consults with the Administrator of Home Health Care Agency regarding need for involuntary termination of employment of a Home Health Care staff member. 2. Informs the Home Health Care staff member that the staff member will be terminated from employment with the Home Health Care Agency. 3. Informs the staff member of the opportunity to participate in an exit interview. 4. Gives a Home Health Care staff member evaluation of employment with the Home Health Care Agency form to the terminating Home Health Care staff member. 5. Requests the terminating Home Health Care staff member to complete and sign a Health Insurance Cover for Terminating Employees. 6. Places the original signed copy of he Health Insurance Coverage for Terminating Employees form in the terminating staff member’s personnel file. 7. Gives a copy of the signed Health Insurance Coverage for Terminating Employees form to the terminating staff member. 8. Documents the involuntary termination of employment on Termination of Employment form. 9. Documents additional information concerning the termination. 10. Submits completed documentation regarding te involuntary termination of employment to the Administrator of Home Health Care. 11. Collects keys, equipment, and other agency property from the staff member prior to issuing the final paycheck to the staff member. 12. Ascertains the status of any outstanding financial obligations to the Home Health Care Agency, such as payments in educational programs, before issuing the final paycheck to terminating staff member. 13. Ensures that the final paycheck is released to the terminating Home Health Care staff member within 48 hours of the involuntary termination of employment 14. Places completed documentation regarding the involuntary termination of employment in the staff member’s personnel file. b.
