MDT Home Health Care Agency, Inc. RESIGNATION OF EMPLOYMENT POLICY: MDT Home Health Care Agency, Inc. licensed, registered, managerial and/or salaried Employees are requested to provide four (4) full weeks written notice of resignation. All other Employees are requested to give a two (2) full weeks written notice to allow adequate time for recruitment and training of new employees to ensure adequate coverage and quality service. All staff will have the opportunity of an Exit interview with the Administrator or Director of Nursing, Clinical Manager. PROCEDURE: · Employees who resign with proper notice will receive all of their unused Paid Time Off (PTO) benefit pay. · Employees who resign without proper notice will forfeit all unused Paid Time Off (PTO) benefit pay according to policy guidelines. · Employees who leave prior to the expiration of the notice period may forfeit unused Paid Time Off (PTO). Pay out will be determined on a case by case basis by the VP of Operations. · PTO benefits shall not be used during a notice period. Prior approvals will be rescinded unless otherwise approved by the Director of Nursing, Clinical Manager. · Employees not following the resignation policy are ineligible for rehire. The Director of Nursing, Clinical Manager or the Administrator may approve exceptions for unusual circumstances. · In certain instances, management may determine it would be in the best interest of all concerned to amend an Employees resignation to be effective immediately. In this instance, the Employee is entitled to unused PTO benefit pay out as outlined in Section A of this policy. · Eligible Employees rehired within ninety (90) days may be reinstated if a position is available with the approval of the Supervisor or Director of Nursing, Clinical Manager. A reinstated Employee will not lose seniority. · An Employee rehired after ninety (90) days of termination will be considered a new Employee. If an employee has borrowed PTO time, the balance due will be taken out of the last paycheck for that employee. Home Health Agency. - - Personnel/Operations Policies B-53
