MDT Home Health Care Agency, Inc. TRANSFER REQUESTS POLICY: Employees that have successfully completed six (6) months of service may request a transfer to another facility or department. PROCEDURE: · An eligible employee may obtain a Transfer Request form from the department supervisor or the Human Resources Department. · An employee must first obtain his/her Supervisor’s signature to process a transfer request. · Employees on a Performance Improvement Plan are not eligible for a voluntary transfer. · Upon approval of the Supervisor, the employee will be interviewed (if applicable) by the new supervisor to determine suitability for a new position. A position must be available to honor the transfer request. · The employee’s work performance and skills will be key factors in the decision making process. · When multiple employees have requested a transfer to the same position, the date of the transfer request and the employee’s seniority with the company will also be factors in the decision making process. · The Supervisor will respond to the employee’s request in a timely manner dependent on the opening date of the position to be filled. · The Supervisor should forward the transfer request form to the Human Resource department to be filed in the Employee’s personnel file. · The Human Resource department will note the employee’s request for transfer and contact the employee prior to seeking outside candidates. · Transfer requests must be approved by the Director of Nursing, Clinical Manager or the VP of Operations.
