POLICY ON MONITORING OF CONTRACTED SERVICES It is the policy of MDT Home Health Care Agency, Inc. that all contracted services shall be monitored closely by this agency, and must be annually evaluated. In keeping with this policy, our Agency and the contracted Professional shall meet for monthly case conferences review to evaluate the patients progress and such evaluation shall be entered into the Patient’s Medical Record.
