MDT Home Health Care Agency, Inc. ORGANIZATION The organization is organized, staffed and equipped to provide professional services to patient on a part-time intermittent basis. The patient will live in a place of residence used as the patient’s home. In addition to these services, there is an emphasis on education and rehabilitation techniques for the patient and his or her family. A. HOME: The patient’s place of residence. This may be his or her own dwelling, an apartment, a relative’s home, a home for the aged, an institution which is defined as an extended care facility. B. HOMEBOUND: Results from a condition which presents to the patient a normal inability to leave the home, and consequently, leaving his/her home would require a considerable and taxing effort. Generally speaking, the patient requires the aid of supportive devices such as crutches, canes, wheelchairs, and/or walkers, the use of special transportation, or the assistance of another person, or leaving his/her home is medically contraindicated; also the aged person who does not often travel from his/her home because of feebleness an insecurity brought on by advanced age. C. PATIENT: A patient is a person who is under a physician’s care from a spell of illness or requires some type of assistance to provide personal care or aid. Clinical records are maintained on all patients. Included will be individual patient records and reporting systems necessary to carry out program evaluation and to meet all applicable professional, administrative, planning and legal requirements. The Agency does not refuse service to, or in any other way discriminate against any person because of race, color, sex, disability, age or national origin. The Agency may provide the following services under physician’s plan of treatment. 1. Registered Nurse 2. Licensed Practical Nurse 3. Home Health Aide 4. Physical Therapy 5. Occupational Therapy 6.
