MDT Home Health Care Agency, Inc. POLICY ON PROFESSIONAL PERSONNEL It is the policy of MDT Home Health Care Agency, Inc. that a group of professional personnel shall be appointed over professional matters of the agency. Such professional personnel shall meet at least once every six months to discuss any concerns affecting the agency. The following shall be the composition of such professional personnel: 1 . The Administrator 2. The Director of Nursing, Clinical Manager 3. A Registered Nurse (RN) 4. A Licensed Practical Nurse (LPN) 5. Therapists and Social Workers 6. A licensed physician (if the Agency hire a Medical Director) 7. An accountant (if applicable) Records of meetings of the professional personnel shall be kept by dated minutes.
