MDT Home Health Care Agency, Inc. Administrator carries out the responsibilities of the Administrator. (See Policy During The Absence of the Administrator Form) 5.In the absence of the Director of Nursing of Home Health Care Agency, a designated Home Health Care Registered Nurse carries out the delegated responsibilities of the Director of Nursing. (See Policy During The Absence of the Director of Nursing Form) 6.Home health care staff members report directly to the Director of Nursing. 7. All care and services provided by our Agency will be provided within the established time frame (initial assessment within 48 hrs, calendar of visit compliance according approved frequency by discipline, discharge planning in place, participation of patients/families, all MD orders followed/updated as needed). All home health care services not directly provided by our agency are monitored and controlled by our supervisory/administrative staff. 8.A written contract is developed an implemented if the home health care agency utilizes personnel under hourly or per visit contracts or utilizes services under negotiated arrangement. The contracts must stipulate the following: a. The understanding that the patients are accepted for care only by the primary home health care agency. b. The home health care services to be provided. c. The necessity to conform to all applicable home health care agency policies and procedures. d. The responsibility for participating in the development of a client’s plan of care. e. The manner in which the home health care services are controlled, coordinated, and evaluated by the primary home health care agency. f. The policies and procedures for submitting client’s clinical and progress notes, for scheduling client’s visit, and for performing periodic client’s evaluation. g. The polices and procedures for determining home health care agency charges and reimbursement. (See Home Services Agreement Form) 9.All home health care agency policies and procedures are annually reviewed by the home health care agency Board of Director. 10.All home health care agency policies and procedures are annually reviewed and approved by the Administrator and Director of Nursing, Clinical Manager of Home Health Care Agency. ADMINISTRATIVE DOCUMENTATION: Home health care staff members will ensure complete concise documentation of services, issues and conditions occurring during the period of services rendered to the client.
