MDT Home Health Care Agency, Inc. PAID TIME OFF (PTO) POLICY: Paid Time Off (PTO) benefits are provided to enable Employees to have time off from work for vacation, holidays, personal business, bereavement and health and well-being. PTO is a gratuity and not a reward for past performance but provided to enable the Employee to prepare for the next year of service. Any Employee who resigns without providing requested notice will not be paid unused PTO benefits. PROCEDURE: Regular full-time Employees who work at least 32 hours or more per week. The accrual rate is determined at the agency discretion. Employees are not eligible for PTO until the completion of the ninety (90) day introductory period unless approved by the Director of Nursing, Clinical Manager. Employees who resign during the introductory period are not eligible for PTO pay out. Holiday pay will be paid in the first 90 days and deducted as PTO is accrued. PTO carryover will be determined at the agency discretion. PTO may be used for vacation, illness, holidays or any reason the Employee may request paid time off. Requested Time off is not reflected as an unexcused absence as related to the attendance policy. PTO will accrue based on regular hours not overtime hours worked. The Employees must specify on the PTO request form the total hours requested, not to exceed eight (8) hours per day unless the employee’s work schedule is a 10 hour shift, then 10 hours per day may be requested. More than forty (40) hours per request will not be granted without approval of the Director of Nursing, Clinical Manager. Any Employee on a leave of absence greater than one month will not accrue PTO benefits during the leave of absence period. PTO will not be granted during the notice period without explicit permission of the Supervisor and the Director of Nursing, Clinical Manager. An Employee requesting more than eight (8) hours of PTO or one (1) workday should submit a PTO request form to the Supervisor or designee at least 21 days in advance. Requests of more than one week should be requested at least 30 days in advance. The Supervisor or designee will give a recommendation to the Director of Nursing, Clinical Manager as to whether the request can be accommodated. In making this recommendation to the Director, the Supervisor or designee should consider whether adequate staffing for continuity of care can be retained during the requested PTO period. Request for PTO shall be evaluated based on a number of factors, including anticipated workload requirements, staffing considerations, seniority and proper advance notice. Employees shall submit a PTO request form to the Supervisor or designee. The Supervisor or designee should then forward the form to payroll for processing. Once processed, PTO requests will be filed on the employee’s personnel file. A copy may be filed with the payroll records. REQUESTING PTO: Employees requesting PTO should do so by completing a PTO request form prior to the requested day(s) off as specified in this policy and submit it to the Supervisor or designee. The Supervisor or designee will review the request with the Director of Nursing, Clinical Manager or designee for approval. The Director or Supervisor shall forward the request to payroll for verification of available PTO. PTO must be accrued prior to being paid. The Human Resources Department is responsible for forwarding the approved/declined request to the Employee.
