MDT Home Health Care Agency, Inc. POLICY ON DIRECTION/COORDINATION/OVERALL SUPERVISION OF PATIENT SERVICES AND CONTRACTS In compliance with the Social Security Act and State guidelines, it is the policy of MDT Home Health Care Agency, Inc. that all services rendered by us to our patients shall be coordinated in a manner that will enhance the overall efficacy and professionalism of our office, as well as provide maximum quality care for our patients by facilitating exchange of information among our staff. To achieve this end, all personnel and nurses/therapist shall be involved in the direct or indirect care of the patient. Our Organizational Chart clearly maps out the coordinated relationship that exists between staff members and patients. Our organization designates a Registered Nurse (The Director of Nursing (DON), Clinical Manager) responsible for direction, coordination, and overall supervision of each type of service provided by our Agency either directly or by contract, the DON, Clinical Manager who is a direct employee, of the agency and who is a graduate of an approved school of nursing and is licensed in this state; who has at least 2 years of home health experience and at least 1 year of supervisory experience as a registered nurse. Must met the educational requirement of be graduate of an approved School of Nursing/College Degree, Licensed in the State of Florida, continually strives to broaden his/her knowledge through formal education, conferences, attendance at workshops, or continues with active participation in professional and related organizations and individual research and reading, completes educational requirements as per state and regulatory agency standards, thorough knowledge and understanding of the functions of the Home Health Agency, competency in general nursing theory, technique and practice. Also the DON, Clinical Manager is responsible for overseeing the professional nursing and home health aide delivery of quality services of the agency. Participating in all activities relevant to the daily operations of the Clinical division. Coordinating and integrating the services provided to patients according to their attending physician’s orders; ensuring the quality and staff needed and that the services are available as needed for agency patients and as presented to the public. Oversees the development of discharge plans for home health patients in cooperation with attending physicians, nurses, and therapists, and when necessary, making suitable arrangements for patient’s continuing care. Ensuring that referral forms, plans of care, service reports, and all other clinical records are properly and promptly written and transmitted to responsible physicians and to contracting agencies including third-party payers. Supervising, guiding, and directing clinical supervisors in the areas of policy and operation, caseload management, and personnel management. Developing and implementing policies and procedures guiding clinical service provision are developed and implemented. Advising the Administrator regarding staffing needs and requirements and resources for services. Providing leadership in development and implementation of a performance improvement program. Oversees the developing, implementing, and evaluating orientation and staff development programs. This person, or a similarly qualified alternate, is available at all times during operating hours and participates in all activities relevant to the professional services furnished. Administrative and supervisory functions are not delegated to another agency or organization. Our policy is that all personnel providing services to a specific patient shall maintain liaison in order to assure that their efforts will effectively complement each other and support the objectives outlined in the respective patient's Plan of Treatment/Care.
