MDT Home Health Care Agency, Inc. "Direct employee" means an employee for whom one of the following entities pays withholding taxes: a home health agency; a management company that has a contract to manage the home health agency on a day-to-day basis; or an employee leasing company that has a contract with the home health agency to handle the payroll and payroll taxes for the home health agency. SCOPE OF SERVICES : A. Services to be provided by our direct employee: 1) Home Health Aide 2) Case Management in cases requiring Nursing and Therapy and/or Nutritional Services, provided by a Registered Nurse or Director of Nursing, Clinical Manager. In cases of patients requiring only nursing, or in cases requiring nursing and physical, occupational or speech therapy services, or nursing and dietetic and nutrition services, the agency shall provide case management by a licensed registered nurse directly employed by the agency. 3) Case Management in cases requiring only Therapy Services, provided by a Registered Physical Therapist, or PTA (not Nursing Services) In cases of patients requiring only physical, occupational or speech therapy services, the agency shall provide case management by a licensed registered physical therapist or Registered Nurse (in coordination with the specific therapy staff) directly employed by the agency. B. Services to be provided under contract, covered under our State License, but will be provided according our marketing abilities in our community and referrals received from all sources like Doctor’s offices, Hospitals, other in-patients facilities, etc. 1)Skilled cases requiring the services of a Registered Nurse, a Licensed Practical Nurse, or a Certified Nursing Assistant. 2)Homemaker/Companion/Sitter services 3) Physical Therapist. 4) Speech Therapist. 5) Occupational Therapist. 6) Medical Social Services. C. Consulting with a Physician Following the acceptance of a case, either the Director of Nursing, Clinical Manager or a Registered Nurse/Registered Physical Therapist shall visit the patient within 48 hours and make an initial assessment of the case to include a well defined patient care plan (including all discipline ordered). In consultation with a physician, the Director of Nursing, Clinical Manager or Admissions Nurse shall draw up a Plan of Treatment which must be approved and signed by a physician. This policy and procedure shall be followed in all cases without exception. D. Patients receiving only physical, speech, or occupational therapy services, or in cases of patients receiving only one or more of these therapy services and home health aide services, case management shall be provided by the licensed therapist, who is a direct employee of the agency or a contractor. E. In cases of patients requiring only nursing, or in cases requiring nursing and physical, occupational or speech therapy, or nursing and nutrition services, the agency shall Home Health Agency Policies A-13
