MDT Home Health Care Agency, Inc. provide case management by a licensed registered nurse directly employed by the agency. F. In cases of patients receiving only dietetic and nutritional services, case management shall be provided by the licensed dietitian/nutritionist who is a direct employee of the agency or an independent contractor. G. If we work with a resident of an assisted living facility, or adult family care home, to provide home health services, we shall coordinate with the facility or home regarding resident’s condition and the services being provided in accordance with the policy of the family or home and if agreed to by the resident or the resident’s representative, we shall retain responsibility for the care and services we provides and avoid duplication of services by not providing care the assisted living facility is obligated, by resident contract, to provide to the patient. Outpatient physical therapy or speech - language pathology services . If we will offer outpatient physical therapy or speech language pathology services we will meet all of the applicable conditions of this part ( Standard: Services under arrangement) and the additional health and safety requirements set forth in § 485.711, § 485.713, § 485.715, § 485.719, § 485.723, and § 485.727 of this chapter to implement section 1861(p) of the Act. Target Patient populations: Miami Dade & Monroe County(ies) has a big population and high portion of them over age 65. Local private health care institutions cover a significant sector of the population, but a significant portion are covered by Medicare, Medicaid and government programs. We believe we have found a niche there for our company to provided needed quality service to an elderly mainly medicare beneficiary patients, but not limited only to them, as our marketing effort also will target HMO/Management care organization, In-patient like Hospice facilities to provide also staffing services to them. Admission Criteria: "Admission" means a decision by our home health agency, during or after an evaluation visit to the patient's home, that there is reasonable expectation that the patient's medical, nursing, and social needs for skilled care can be adequately met by the agency in the patient's place of residence. Admission includes completion of an agreement with the patient or the patient's legal representative to provide home health services.
