A physicians order is obtained if the assessment indicates additional disciplines are needed. The qualified professional doing the assessment functions as follows: (i) Assists the physician in evaluating level of function; (ii) Helps develop the Plan of Treatment (revising as necessary); evaluate outcomes of care, (developing and revising the plan of care in consultation with the physician and other care-team members) (iii) Prepares clinical and clinical notes; prepare for discharge, participate in discharge planning (iv) Advises and consults with the family and other agency personnel; (v) Participates in in-service programs. (vi) Conducts regular or OASIS assessments accurately, according to instructions in Chapter 8 of the OASIS Implementation Manual, and corresponding to documentation contained elsewhere in the assessment note. (vii) Providing therapeutic treatments and evaluation of equipment needs as applicable (viii) RN supervising HHAs, LPNs, RPT Supervising Therapy Assistants and Home Health Aides as appropriate, ROT supervising OTA and HHAs. a) Policy on Supervision of Physical Therapist Assistant, Speech Therapist assistant, and Occupational Therapy Assistant. In accordance with the policy of this agency, services provided by a qualified physical therapist assistant or occupational/speech therapy assistant are furnished under the supervision of the agency's qualified physical, speech, or occupational therapist every 30 days. The physical therapist assistant or occupational therapy assistant functions as follows: (i) Performs services planned, delegated and supervised by the therapist; (ii) Assists in preparing clinical notes and progress reports; (iii) Participates in educating the patient and family; (iv) Participates in in-service programs. The Home Health Aide (HHA or CNA), may also be Supervised by the Registered Physical/Occupational Therapist if the case only involves Therapy and Aide Services. b) Policy on Supervision of Speech Therapy Services. It is the policy of this agency to provide speech therapy services only by or under the supervision of a qualified speech pathologist or audiologist every 30 days. c)Therapy Documentation requirement (therapy-specific assessment requirements): To ensure therapy services are effective, at defined points during a course of treatment for each therapy discipline for which services are provided, a qualified therapist (instead of an assistant) must perform the ordered therapy service. During this visit, the therapist must assess the patient using a method which allows for objective measurement of function and successive comparison of measurements. The therapist must document the measurement results in the clinical record.
