MDT Home Health Care Agency, Inc. POLICY ON SUPERVISING VISITS POLICY: It is the policy of MDT Home Health Care Agency, Inc. that a Registered Nurse or Registered Physical/Occupational Therapist shall make Home Health Aide supervising visits to each patient receiving skilled services at least one time every two weeks (14 days cycle), and at least 1 a month (every 30 days) to Licensed Practical Nurse (LPN). We shall obtain the patient’s permission to send a registered nurse into the home to conduct a supervisory visit. Supervisory activities include, but are not limited to: A visit to the patient’s home by the RN, with or without the LPN/LVN present at least every 30 days Patient record reviews, conferences, ongoing communication Collaborative care planning Specific assignments made by the Registered Nurse for the Licensed Practical Nurse For Waiver, Home Health Aide without Skilled Services, Personal Care only, and/or Homemaker/Companion Services, at least one time every 60 days, according Patient’s needs, directions. Our Agency shall provide RN supervisory visits to non-Medicare/Medicaid Patients, receiving Aide services in accordance with Patient’s direction and approval. Also we encouraged PRN supervisory visits whenever necessary. It is recommended that the RN make such a visit in such a way that the Home Health Aide will be present at some visits while at other visits, the RN should make the visit at a time the HHA is not physically present. At each visit, the RN must sign the Communication/Supervisory Sheet to show the date and time of visit, remarks or instructions, and her signature. The Communication/Supervisory Sheet shall be turned into the agency's office once a week. Also: Reviews and update the Care Plan/Medication Profile, provides demonstration request and evaluates the return demonstration, denotes client’s satisfaction with field staff providing services. A Therapy Assistant performs services planned, delegated, and supervised by the Registered therapist. Supervision includes regularly scheduled patient record reviews, conferences and a visit to the patient’s home by the Therapist, with or without the Therapist Assistant present. The supervision of a Physical/Occupational/Speech Therapist assistant (PTA/OTA/STA) shall require on- site supervision by the registered Physical/Occupational/Speech therapist (PT/OT/ST), at least monthly, every 30 days, he/she shall be accessible at all times by two way communication, which enable the PT/OT/ST to respond to an inquiry when made and to be readily available for consultation during the delivery of care, and shall be within the same geographic location as the assistant. We maintain a process that includes a procedure for assessing the Social Worker Assistant's practice and a method for assuring that patient needs are met. The master's degree prepared Medical Social Worker approves the plan of care periodically and provides clinical supervision at least once every 30 days. Supervision will be done by means of case conferences, joint visits, or both, depending on the needs of the patient and skills of the assistant. The supervision of the assistant is documented. The Director of Nursing, or Clinical Manager, monitors supervisory visits to: a. Confirm that the supervisory visit occurred b.
