MDT Home Health Care Agency, Inc. MISSION STATEMENT POLICY Home health care services are guided by the home health care agency mission statement. PURPOSE 1. To ensure quality of home health care services to clients. 2. To provide continuity of home health care services for the clients. PROCEDURE A mission statement is used as the basis for developing and maintaining the agency’s home health care services. The Board of Director will review our mission statement regularly, and approved it at least every 36 months, and communicates it to all personnel. MISSION STATEMENT The Agency's mission is to provide professional and paraprofessional services to clients in their homes assisting them to achieve the highest level of potential in their day-to-day self-care activities. We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional's point of view. VISION STATEMENT To be one of the leading providers of a wide range of quality home health care services, recognized for enduring dedication to provide innovative, professional, and compassionate care to the communities we serve.
