MDT Home Health Care Agency, Inc. CLIENT’S SUMMARY REPORTS POLICY Home Health Care staff and interdisciplinary team members who provide Home Health Care Services and maintain liaison with each other. PURPOSES 1.To ensure coordination of client’s Home Health Care Services. 2.To ensure that the efforts of the Home Health Care Staff and interdisciplinary team members to provide services effectively complement one another and support the objectives outlines in the client’s Plan of Care. PROCEDURES 1.Pertinent factors regarding a client are drawn from relevant clinical and clinical notes and submitted on a Summary report to the client’s physician. 2.The Home Health Care Registered Nurse or other Professional staff: a. Completes a Summary Report of services provided, patient’s status, response to treatment, goals met or un-met and outcomes reached - also Medical/Verbal Orders and Request for Home Health Services for each client at least every 60 days (if patient will be recertified) b. Mail or fax a copy of the completed Report of Professional Worker to the client’s attending physician, physician assistant, or advanced registered nurse practitioner, acting within his or her respective scope of practice, at lease every 60 days or on discharge date. c. Places the original or electronic copy of the completed Report of Professional Worker in the client’s clinical record at least every 60 days or on discharge date. 3.The Director of Nursing, Clinical Manager of Home Health Care monitors summary reports to ensure: a. Adequate documentation b. Accuracy of content c. Their time-lines d. Appropriate mailing of the report Home Health Agency - - Skilled Professional Services C-24
