MDT Home Health Care Agency, Inc. GOALS AND OBJECTIVES POLICY: The approach of the home health care agency in maintaining quality home health care services is guided by the objectives of the agency. Goals are established as part of the strategic planning for the organization. PURPOSE 1. To ensure that the quality of the agency’s home health care from clients is consistent with the agency’s objectives. 2. To provide continuity of home health care for clients. 3. To provide qualified skilled and non-skilled services focusing on preventative, rehabilitative and therapeutic care in an institution or home environment. PROCEDURES Leadership utilizes the goals to monitor effectiveness of the organization. The goals are communicated to all personnel. 1. The home health care agency implements programs and services to achieve the following objectives: a. To provide comprehensive quality client and client’s family center home health care in the client’s place of residence. b. To address the client’s unique physiological, safety, psychological, self esteem and self-actualization needs in a client’s plan of home health care. c. To provide home health care services that promotes the client’s value of life by: 1. Minimizing negative client’s illness and disability outcomes. 2. Maximizing potential client’s level-of-independence outcomes. 3. Restoring, maintaining and promoting client health. d. To assist the client in search for meaning in personal life experiences surrounding the client’s illness or disability, as appropriate. e. To assist the client to make choices and decisions about home health care services relative to the client’s self-defined unique needs. f. To encourage the client to collaborate actively with home heath care staff members and other health care providers regarding the provision of home health care services. g. To involve the client- family in the client’s plan of care whenever possible. h. To advise the client-family regarding community support services, as needed. To provide health care education for the client, for the client-family and the community. 2. Efforts and strides toward continued growth for field staff are achieved through: a. Maintaining an effective recruitment program in expectations of growing needs for nursing personnel and allied health care personnel. b. Implementing methods of screening applicants and evaluating their progress regularly in order to provide quality personnel. c. Providing staff development opportunities and programs, including orientation and programs concerning current practices in nursing and allied health care. d. Providing current and accurate personnel records, including verification of current license/certification (as applicable) and health records. e. Maintaining open communication with clientele, employees, contractors, third party payers, and physicians via frequent contact in order to achieve quality and cost - Home Health Agency Policies A-17
