The patient/responsible party has a right to be involved in the development of the plan of care and any changes in that plan. The degree of involvement may vary depending on the status of the patient, by we encourage that they speak up their feelings, ideas of better implementation of the physician’s orders and the outcomes of the treatments. At a minimum, the patient or responsible party agrees to the plan of care prior to the beginning of services and as subsequent changes occur. The Patient’s Physician will be notified when there is a need to alter the plan of care immediately after the Agency’s is notified of the need of adjust/modify the Plan of Care, and asking for approval, a Modify Order reflecting those changes must be signed by the Patient’s Physician within 30 days, and copy of the modified plan will be distributed to all involved discipline in patient’s care.
